I got Citizen Pain during a collect mission where I started in the south room of Crypt of Sul. Also got him on a kill mission starting in NW room. Collect mission is crap though, since every room is full of mobs and I killed too many barren rooms until I found the rooms with the drops.

If you get the "collect" variations of the mission might as well just drop it and get a new one. The chance for named only seems to happen in the variation where you initially spawn in the NW room, not the South room.

It casts a spell called : Citizen's Pain. Better burn him fast.
AE Range: 2000
Decrease HP by 12,000 per tick
Decrease Chance to Block by 50%
Decrease Mana Regen by 146,700 per tick at Lvl 105
Decrease Endurance by 119,400 per tick at lvl 105

In the #Destroy corpses mission, a reserved worshipper pops in the NE room (first room). Usually on the left in a stable.
